The new plan is expected to be approved and to take effect in January, although it will be open to public comment for 60 days.<br /> <br /> Medicare’s plan comes as many patients, families and health providers are pushing to give people greater say about how they die — whether that means trying every possible medical option to stay alive or discontinuing life support for those who do not want to be sustained by ventilators and feeding tubes.<br /> <br /> “We think that today’s proposal supports individuals and families who wish to have the opportunity to discuss advance care planning with their physician and care team,” said Dr. Patrick Conway, the chief medical officer for the Centers for Medicare and Medicaid, which administers Medicare. “We think those discussions are an important part of patient- and family-centered care.”<br /> <br /> Dr. Conway said a final decision on the proposal would be made by Nov. 1. The plan would allow qualified professionals like nurse practitioners and physician assistants, as well as doctors, to be reimbursed for face-to-face meetings with a patient and any relatives or caregivers the patient wants to include. Dr. Conway said the proposal did not limit the number of conversations reimbursed.<br /> <br /> “The reality is these conversations, their length can vary based on patients’ needs,” he said. “Sometimes, they’re short conversations — the person has thought about it. Sometimes, they’re a much longer conversation. Sometimes, they’re a series of conversations.”<br /> <br /> Major medical organizations endorsed Medicare’s proposal. The National Right to Life Committee opposed it on grounds that it could lead to patients’ being pressured to forgo treatment.<br /> <br /> The reimbursement rate paid under the proposal and other details will be determined after public comments are received, Dr. Conway said. People covered by Medicare account for about 80 percent of deaths each year.<br /> <br /> Because Medicare often sets the standard for private insurers as well, the new policy would prompt many more doctors to engage patients in such discussions about their preferences. Some private health insurance companies have recently begun covering such advance care planning conversations, and more are likely to do so once Medicare formally adopts its new rules.<br /> <br /> Efforts to support end-of-life planning were derailed in 2009 during the debate over the Affordable Care Act. The next year, Medicare decided through its regulatory powers to allow coverage for “voluntary advance care planning” in annual wellness visits. But soon after, the Obama administration capitulated to political pressure and rescinded that part of the regulation.<br /> <br /> As a big part of the American population ages, and more people live longer with grave illnesses, a growing number of people want to be able to talk over options with their doctors — deciding, for example, whether they want to die at home or in the hospital, or under what circumstances they would want life-sustaining treatment.<br /> <br /> Last September, a national nonpartisan panel of medical, legal and religious leaders issued a far-reaching report saying that the country’s system for dealing with end-of-life care was seriously flawed and should be overhauled. Among its recommendations was that insurers reimburse health providers for advance care planning conversations.<br /> <br /> Filling out advance directives can help, and some states have created central databases of these forms so they are easier for a patient’s doctor or specialist to find in moments of crisis. But end-of-life experts and the members of the national panel, said simply checking boxes on a form were of limited value because many patients needed several conversations to consider their options and to talk them over with their families.<br /> <br /> “It’s a huge step forward,” said Dr. Diane Meier, a member of the national panel and the director of the Center to Advance Palliative Care. “I think it’s great news that Medicare, the major payer for health care in the U.S., is now formally recognizing that advance care planning is worthy of its attention and reimbursement and that in fact is a way to restore power and control to patients.”<br /> <br /> But, she said, “It’s not enough to pay for it. You have to make sure people know what they’re doing and are well trained.”<br /> <div> <br /></div> <div> <a href="http://asmclk.com/click.php?aff=34082&camp=59498&sub1="><img border="0" src="http://asmclk.com/creative.php?id=110270" /></a><br /> <br /> Dr. Meier said that in the final policy, Medicare should identify and require documentation of specific components of end-of-life discussions. “What you don’t want to have is a check-box,” she said. “ ‘Yes I had a conversation — I’m billing for it,’ should not be enough.”<br /> <br /> Burke Balch, the director of the National Right to Life Committee’s Powell Center for Medical Ethics, said his organization objected to Medicare’s plan under current circumstances. “We feel there is pervasive bias against treatment that is occurring in advance care planning that involves nudging individuals to reject lifesaving medical treatment,” he said. He said such pressure on patients was “motivated in large part by a desire to cut health care costs.”<br /> <br /> Mr. Balch said his organization was trying to find a congressional sponsor for a bill that would instead direct Medicare to produce materials about end-of-life decisions that reflect a range of views, including those “concerned with protecting people’s right to treatment.”<br /> <br /> In the rural upstate New York community of Dundee, Dr. Joseph Hinter Berger, a family physician who has been conducting end-of-life discussions with his patients at no cost, took a different view. He praised several aspects of the plan and suggested also covering social workers and other trained providers.<br /> <br /> “I think what it will mostly do is decrease confusion at the end of life,” he said. “It will potentially decrease unnecessary use of resources because it will be very clear what the patient did and did not want. And it should also make it easier for physicians and other caregivers to make decisions because they can say, ‘We all know what Mildred’s wishes were. There are so many facets of Yoga, that one life span, is just not enough time to learn it all.<br /><br />It's not a race, but it is a journey. You will find friends, colleagues, and students who are on the same path. This makes giving, receiving, and sharing a wonderful thing along the way.<br /><br />Time<br /><br />You will have time to stop, think, breathe, relax, or meditate. You can always fill your plate beyond its limits, but you no longer have to.<br /><br />You can determine whether or not you will be stuck in traffic during rush hour. You decide what hours you will work and what days you have off. You will come to the realization that your time is your own.<br /><br />Independence<br /><br />Everyone wants control of their own life, but very few achieve it. Being in business for yourself, can help you control your own destiny and that of your family.<br /><br />Sure there are limits to what one person can accomplish, but it is better to try than to have never tried.<br /><br />Success<br /><br />No matter what you want, if you write down your short-term and long-term goals, you will make great progress toward them.<br /><br />You should keep these goals in a place where you can see them daily and visualize yourself accomplishing your goals. You should be specific about time frames and ethical methods used to meet them. You can even use them in meditation.<br /><br />Review your long-term goals at least once every season and every year. Review your short-term goals daily. You will see yourself make rapid success in this way.<br /><br />Lastly, goals do not have to be material at all. For example: You may want to start teaching Yoga in a year, and the following year, get a part-time Yoga teaching position. Rules and discipline policies are usually in place so that each child can learn Yoga and get the most out of their experience.<br /><br />For your child's safety at home: Parents who are not familiar with Yoga, should not let their "beginner Yoga children" practice anything beyond beginner techniques, without a competent Yoga instructor's approval. Once your child's Yoga instructor gives approval of home practice, you are fine. This one safety factor should encourage parents to learn Yoga, practice with their children, and live a healthy life.<br /><br />A smaller kid's Yoga class is actually better than a larger one. This insures that your child's Yoga instructor can pay close attention to each student during practice session.<br /><br />Price should not be the top factor in choosing your child's Yoga teacher. Would you choose the cheapest dentist, clothes, or car? Like ballet, the promise of poise, grace and flexibility encourages much interest. Yet if yoga is like any other exercise, it is only at first glance. Underneath the coveted health and fitness benefits of modern yoga practice are the roots of a great spiritual tradition blending cultures and religions.<br /><br />Unfortunately, through the westernization of yoga, we have lost an essential component of this peaceful practice. Though the ritual remains intact the meaning has gone astray. Where previously the postures of yoga practice were merely a branch of a tree; today they are viewed by western society as the tree itself. How do we bring meaning to our poses?<br /><br />Modern Yoga's Roots and Benefits<br /><br />Though the development of yoga cannot be pinned to an exact year, the discovery of the Indus seals, which show figures in the classic yoga asana (posture) of lotus pose, trace yoga back to at least 3000 BCE. At this time the Vedas were being written, which today's yoga postures are derived from. These gave birth to Vedic yoga, which accommodated the ancient Indians fixation on ritual and sacrifice. We see evidence of the importance of sacrifice in the yogic corpse posture. Lying as though we were placed in a coffin, this represents the ultimate sacrifice-that of death. Though seemingly morbid, corpse posture is one of hope when we understand that according to the Vedanta sutras, death results in liberation<br /><br />Sacrifice was meant to join the material and the physical, and create the longed-for "union" that defines the word Yoga. The Vedanta sutras (vs.4:4,13-14) declare that the liberated soul is not materially motivated.. By asking us to free ourselves from the bondage of material motivation, compassion requires us to be selfless. Surely, this giving is an exercise in compassion. Even still, the modern practice of yoga facilitates this. Through postures and stillness, we alter our consciousness and therefore change our perspective. In our new realization of others as being part of the cosmic whole, we feel that in giving to them we are also giving to ourselves.<br /><br />The ancient philosophy of yoga saw its postures as part of a greater whole. Thousands of years ago during the time of Astanga yoga, posture practice was one piece of a more important whole. Astanga yoga, which originated during Vedic India, was derived of eight branches; yama (control and discipline), niyama (rules, methods and principle), asana (posture), pranayama (focused breathing), prathyahara(avoidance of undesirable action),dharana (concentration), dhyana (meditation) and samadhi (contemplation). In contrast, most contemporary yoga focuses on postures and uses breath work as a small component or an afterthought. Though admittedly, the current vision of yoga over-emphasizes asana, it is extremely important and has enormous benefits. The medically studied benefits of yoga include the following: Stress reduction, improved muscle strength and tone, increased energy and flexibility, improved balance and coordination and a reduction in depression. <br />. <br />Moving with Compassion<br /><br />Through most of asana practice, we are unconsciously engaging in physical metaphor. Many yoga postures are named after and imitate the living world; tree pose, eagle pose, frog pose, cat pose. By developing postures that imitate the stance of animals, the vedic seers may have been seeking not only to embrace the qualities of these animals, but to formulate compassion for them. <br />The way that compassion serves as a partner to yoga's goal of liberation can be understood through reading the ancient yogic texts. Understand these vedas; whether they be the Rig Veda (knowledge of praise),Yajur-Veda (knowledge of sacrifice), Sama Veda (knowledge of chants), and Atharva-Veda(knowledge of atharvan), is likelier while deep in meditation. Understanding the vedic sutras, we are permitted to experience a bliss unknown through material grasping.<br /><br />In this state everyday experiences fade away and a greater perspective unfolds. Over time, meditation also allows us to become more intuitive and receptive. This opens us up to others, enhancing our compassionate nature. This experience has been discussed by Eastern sacred-text expert "H.P. Blavatsky's in "The Voice of the Silence." Blavatsky writes: "Compassion is no attribute. IT is the LAW of LAWS--eternal Harmony, Alaya's SELF; a shoreless universal essence, the light of everlasting Right, and fitness of all things, the law of love eternal."<br /><br />In order to experience compassion for others we must first extend it to ourselves. Straining too forcefully in a pose is counter to compassion. Why? Yoga teaches us that we are all connected, so when we hurt ourselves this pain eventually reaches others. Instead we must strive for gentle self-acceptance, competing with nobody-not even ourselves. This is essential to a rewarding experience of yoga.<br /><br />Postures for Peace<br /><br />By the time of Patanjali's yoga sutras, which were written near the beginning of the common era, we begin to see a discussion of the more practical aspects of yoga. Posture is discussed (be it mainly for meditative purposes), as is concentration of the mind during this exercise. In the Yoga-Sutra, Patanjali presents relaxation as the very essence of yoga practice. He teaches us that posture should be steady and comfortable .This sentiment is reflected in the postures (asanas) of today's practice. The physical dimension of yogic exercise requires us to have compassion for our limits. We are never asked to push, instead only to release. Mercifully, our small efforts are graced with us connecting to a life-force that is divine and encompassing.<br /><br />Asanas urge us to see our body as divine, and to nurture health in this mortal temple. Yogic adepts understand that their body is flawed, however slender and toned it may look externally. This acknowledgement leads to less judgement of other's bodies. However pleasing to the eye a yogis shape may be, the same vedic texts that encourage the practice of yoga for health, also remind us that true "liberation" comes from being free of the cycle of rebirth-free of the physical form.<br /><br />Yogic postures work in contrast to the western notion of exercise. Here we see exercise as an end, such as an end to overweight and fatigue. Yoga is different. While in most forms of exercise the physical results are the sole goal, in yoga the soul is the goal. The ancient tradition of yoga exercise stands apart in its doctrines. The ancient yoga texts insist that the mind and spirit are more important than the physical body. While many other eastern forms of mind-body fitness also encourage this awareness, no other physical practice has the ultimate goal of union with the divine. In yoga, the process of attaining this union is as important as the actual attainment.<br /><br />Yoga practice is not a means to an end. It is an end in of itself. Even distinguished from vedas and sutras, the modern practice of yoga posture is a beautiful and calming pursuit. Though modern yoga practice makes little mention of the scriptures that it is based on, the experience of union and compassion can be woven into each pose.
